Is your suppressor on straight? Unless your gunsmith checked at the time you got your barrel threaded... you might not know! In the video, you'll see just how far from straight some suppressors can be. I was at TS Customs shop getting a barrel threaded, and just for the heck of it, we threaded the suppressor on and checked it for concentricity after getting the threads cut. What we found was not confidence inspiring in regard to how tight of tolerances some suppressor manufacturers are keeping. Point in fact, one was so bad that we're certain there would have been a baffle strike if it were fired in that condition.
